To Whom It May Concern:
Structure located at 1523 Holly Ave., Arcadia, CA was uncompleted and left exposed to
weather for over a year. When structure expose to weather for a long period of time,
some of the woods are warp and nails are rust. Below are lists of items that need to be
replaced:
1. All exterior plywoods with rusted nailing need to be replaced.
2. Any wood studs or posts that are warp need to be replaced.
3. Any joists or beams that are warp need to be replaced.
4. Interior floor diaphragms do not need to be replaced. However, floor
diaphragms need to be blocked at certain location. See revised S-2 with
clouded area and delta 1.
5. Provide additional lag screws at sole plate. See revised S-2 with clouded area
and delta 1.

Based upon the observations, the rough grading of the lot(s) listed above have been
completed in conformance with plans marked "APPROVED" by the City. The work
includes but is not limited to the following: grading to approximate final elevations;
staking of property lines; location and gradient of cut and fill slopes; location, cross-
sectional configuration and flow-line gradient of drainage swales and terraces; berms

Dear Mr. Zialcita:
This report presents the results of our field density tests performed on the building pad of the
proposed residence after existing house has demolished and soil compaction was done. Elevated
building pad was constructed for proposed building foundation. Site condition, field and
laboratory test results and post-grading recommendation are presented as follows:
SITE CONDITION
The proposed site lies within a residential area located at 1523 Holly Avenue, Arcadia,
California(Figure 1). The property exhibits a level topography.
The property investigated is located on the south side of Camino Real in the City of Arcadia,
California(T.G. pg. 597, B1) and north side of Winnie Way. It is rectangular in shape and has a
street frontage of approximately 60 feet and an average depth of 131 feet.
Onsite loose and/or disturbed soils within majority of the proposed building area were scarified
approximately 4'-5" and compacted to 90% minimum relative compaction. Onsite and import

soils were used as fill materials. On site soils consisted of dark brown clayey silt(ML).
Imported soil consisted of silty sand (SM). Imported fill was mixed with on site materials.
The compaction test location are presented on the map (Figure 1) attached herewith.
LABORATORY TESTING
Laboratory minimum dry density and optimum moisture content for onsite and import soils were
determined according to laboratory standard ASTM D-1557-04. The results are presented in
Appendix A of this report.
FIELD DENSITY TEST
Field density tests were performed using the Sand Cone Method (ASTM D-1556-04). Field
density test results are presented below in Appendix A. Approximate locations of the tests are
shown on the enclosed Compaction Map (Figure 1).
Field density tests were performed at random locations to check compaction effort provided by
the contractor. Test results exceeded minimum required relative compaction of 90 percent. Test
results herein are considered representative of the compacted area.
POST-GRADING RECOMMENDATIONS
All utility backfill should be brought to near optimum moisture content and then compacted to
obtain a minimum relative compaction of 90 percent of the laboratory standard.
Soils generated from footing excavations should be compacted to 90 percent minimum relative
compaction, whether it is to be placed in landscape areas or within areas to be improved. The
material must not alter positive drainage patterns away from the structural areas.
All trench excavations should conform to CAL-OSHA and local safety codes.
REGULATORY COMPLIANCE
Cuts, fills or processing of original ground under the purview of this report have been completed
under th observation of and with selective testing by Leedco, and are found to be in compliance

I have been requested by Mr. Kruckeberg to respond to your letter dated November
28,2007.
Development As discussed in person on at least two separate occasions I have explained the plan
check history for 1523 Holly, Plan Check#06-131. On April 7, 2006 you submitted
Services plans for review for a new single family dwelling at the above referenced address. The
Department plans were routed to the various City departments and were ready to be picked up for
revisions on April 21, 2006. You picked up the plans with corrections on April 21,
2006 and were advised to make all necessary corrections and return them to the City
for further review. On August 17, 2006 a courtesy expiration letter was sent to you
Don Penman reminding you that the plan check is only valid for 180 days and if needed you could
Assistant City Manager/ apply for a one time only extension of 180 days. After you requested the extension in
Development Services
Director writing, an extension was granted for an additional 180 days. As indicated clearly in
the letter only one extension can be granted for the plan check. The extension was
approved until April 7, 2007.
The resubmittal for the second review was received by the City on March 26, 2007
just twelve days prior to the expiration date of April 7, 2007. Based on the fact that
you did not resubmit the corrected plans for over eleven months after you received the
original corrected set, it made it virtually impossible for you to obtain approval before
the expiration date. The plans were routed to the various departments for the second
time and ultimately they expired on April 7, 2007.
On November 13, 2007 you came in and picked up the plans after over seven months
of no communication from your office, since the March 26, 2007 resubmitted date. At
which time you were advised the plan check was expired. Since November 13, 2007 it
had been approximately seventeen months since the original submitted date of April 7,
2006. All of the plan check documents and the extension letter clearly indicate the
time limitation of 180 days with one 180 day extension only, for a total of twelve
months. The plan check has exceeded the 180 day limitation and the 180 day plan
check extension and is now null and void.
