Dear Mr. Mak:
At its June 26, 2018 meeting, the Planning Commission voted 5-0 to
approve an Amendment to Conditional Use Permit No. CUP 15-02 at 56
E. Duarte Road, subject to the amended conditions listed in the attached
Resolution No. 2016.
There is a ten (10) day appeal period for this application. For an appeal
to be filed, a completed Appeal Application form must be submitted to
the Community Development Division along with a $600.00 appeal fee
by 5:30 p.m. on Monday, July 9, 2018.
Approval of the Amendment to Conditional Use Permit No. CUP 15-02
shall not be of effect until the appeal period has passed and the property
owner and applicant have executed and filed the enclosed Acceptance
Form to indicate awareness and acceptance of these conditions of
approval.
