A procedure five cleanup was required once 5M staff encountered damages located at the site prior to our arrival. All abatement has been completed per the regulations. Scope of Work: Cardinal Environmental provided a scope of work that included: • Notification of agencies • Create abatement work area • Procedure five clean-up • Non-hazardous waste disposal • Hazardous waste disposal Abatement Procedures As applicable: Polyethylene sheeting was used to create a containment and workspace suitable for abatement. A three-stage decontamination unit was installed at the work area entry. The area was surrounded by signage barrier tape.Negative pressure (a slight vacuum)was maintained in the areas by HEPA filter-equipped negative air machines as needed. The use of an airless was incorporated into the work plan to suppress dust in the area. All personnel entering work area were equipped with HEPA filter respirators and protective clothing. At the time of the excavation activities, the Property was under demolition. The Property is proposed for redevelopment by Olson Urban Housing, LLC as a residential community with townhomes, detached single-family homes, and two commercial retail buildings along Las Tunas Drive. Surrounding properties consist of a church on the adjoining property to the north, residential properties and medical offices on the adjoining property to the west, a Starbucks coffee shop adjacent to the southeast, retail properties to the south of East Live Oak and Las Tunas Drive, and a gasoline service station further to the east across South Santa Anita Avenue. A Property location map is illustrated on Figure 1. A Property Details Map illustrating the main features of the Property is provided as Figure 2. Property Background Stantec completed a Phase I Environmental Site Assessment (ESA) for the Property in February of 2017,a Phase II ESA in February if 2017, and an additional investigation under the oversight of the Los Angeles County Fire Department (LACFD) in April of 2018. These investigations identified residual pesticide impacts to soil from former agricultural uses, at one location in the southwest corner of the Property, at concentrations above the United States Environmental Protection Agency's Regional Screening Level (RSL) for residential use. The detected concentration of the pesticide 4,4-Dichlorodiphenyldichloroethylene (DDE) was laterally and vertically defined to consist of approximately 200 cubic yards of DDE-impacted soil in the area shown on Figure 2. The Phase II investigations also detected tetrachloroetheylene (PCE) in soil vapor from a past dry cleaning operations in the north-central portion of the Property. The detected pesticide concentration in soil and the PCE in soil vapor were above their regulatory thresholds for residential use-which is the proposed future use of the Property. Accordingly, the pesticide impacts to soil and the PCE impacts to soil vapor were identified as recognized environmental conditions (RECs) under ASTM E 1527-13. - Based on the above findings, Stantec submitted a Remedial Action Plan for Soil and Soil Vapor Impact(RAP), dated May 2,2018, to LACFD for review and approval. The RAP was approved by LACFD for implementation in its letter dated September 5,2018. The RAP proposed the excavation of pesticide-impacted soil above the residential RSL (former boring HA-4; see Figure 2) and the performance of post grading soil vapor sampling and/or mitigation of PCE vapor concentrations through the installation of vapor barriers with sub slab passive venting systems. It should be noted that mitigation of the soil vapor impacts from PCE will be performed at a later date,once grading is completed, and is therefore not discussed further in this report. The remainder of this report presents the excavation and verification sampling results related to the completion of the pesticide-related remedial excavation activities. 5 Stantec • PESTICIDE EXCAVATION COMPLETION REPORT Completion of the Remedial Excavation and the Soil Verification Results On March 20 and 21, 2019,Stantec provided oversight for the excavation of pesticide-impacted soil at previous boring HA-4, where 4,4-DDE was detected above residential RSLs (see Figure 2). Soil samples were collected for the sidewalls and bottoms of each of the two excavations. The resulting excavation measured 45 feet by 40 feet by 3 feet deep. Table 1 provides the historical results of all soil samples collected on the Property for pesticides. Table 2 provides the results of the excavation verification samples. The verification sample results reported concentrations of 4,4-DDE at.a maximum of 0.0052 mg/Kg. No other pesticides were detected in the excavation verification samples. The concentrations of 4,4-DDE that were detected in the verification samples are well below the US EPA residential RSL of 2.0 mg/Kg. Conclusion The verification sampling results confirm that the excavation was successful in remediating pesticide concentrations in soil at the Property to below residential RSLs. As a result, Stantec is requesting that the LACFD review the results and provide written approval that site cleanup objectives for pesticides have been met and that grading can commence at the Property. Stantec . ii PESTICIDE EXCAVATION COMPLETION REPORT INTRODUCTION March 29,2019 1 .0 INTRODUCTION This report documents the methodology and results of a remedial soil excavation to remove detected pesticide impacted soil from the subject Property.Stantec was engaged to collect and analyze verification soil samples from the resulting excavation completed on the property located at 17 Las Tunas Drive in the City of Arcadia, California (the "Property"). The completed work was conducted in accordance with Stantec's Proposal for Remedial Excavation Oversight and Sampling,dated January 28,2019. The scope of work and the remedial excavation results are described in subsequent sections. 1.1 SITE DESCRIPTION AND OPERATIONS The Property is a 5-acre parcel of land formerly developed with a multi-tenant commercial/retail building and asphalt parking lot at 17 Las Tunas Drive in Arcadia, California. The area is located within the Peninsular Ranges Geomorphic Province,which includes northwest-southeast trending mountain ranges and valleys that have been developed by the San Andreas Fault system (California Geological Survey [CGS], 2002). The stratigraphy underlying the Property consists primarily of recent-age alluvial deposits (CDMG, 1962). Phase II activities performed at the Property have found soils to be sand- silt mixtures to the maximum depth of 55 feet bgs. The closest mapped recently active fault is the Raymond Fault located approximately 1.5 miles northwest(CGS,2010).According to official maps of California,the Site is located within an Alquist- Priolo (AP) Earthquake Fault Zone boundary and a liquefaction zone (CDMG, 2000). The Property is located within the San Gabriel Valley Groundwater Basin.The basin is constrained by the Raymond fault to the north,the Merced and Puente Hills to the south and west, and to the east by Chino and San Jose faults. Several aquifers are present in the basin and water-bearing units consist of semi consolidated alluvium (Department of Water Resources [DWR], 2004). Stantec 1.1 PESTICIDE EXCAVATION COMPLETION REPORT INTRODUCTION March 29,2019 Groundwater sampling conducted roughly a quarter mile to the southwest of the Property reported groundwater depth at approximately 140 feet bgs (DPW, 2013). The intent of this report is to present the excavation and verification sampling results related to the pesticide excavation and to request that LACFD provide written approval that site cleanup objectives for pesticides have been met and that grading can commence. 5 Stantec 2.1 PESTICIDE EXCAVATION COMPLETION REPORT SCOPE OF WORK March 29,2019 3.0 SCOPE OF WORK 3.1 PRE-ASSESSMENT ACTIVITIES Prior to the commencement of fieldwork activities Stantec made the following preparations: • • In accordance with federal OSHA regulations (29 CFR, Section 1910.120), Stantec developed a Site-specific Health and Safety Plan (HASP) for the subject property. All Stantec personnel and subcontractors associated with the project were required to be familiar with, and comply with,all provisions of the HASP. 3.2 EXCAVATION AND FIELD OVERSIGHT Stantec provided the services of a field geologist or engineer to supervise and direct all onsite activities. On March 20 and 21, 2019, Stantec personnel was onsite to oversee the removal of pesticide soils from the remedial excavation and to collect verification samples within the excavation. All remedial excavation work was conducted under the supervision of a State of California registered professional and included the following: • Onsite delineation of the excavation area measured to be 45 feet (north-south) by 40 feet (east-west); • Excavation and direct loading of approximately 200 cubic yards of soil; • Collection of thirteen (13) verification samples from the excavation (four bottom samples and nine sidewall samples) for chemical analysis of organochlorine pesticides (OCPs) by EPA Method 8081A. The final excavation dimensions were measured to be 45 feet by 40 feet by 3 feet in depth. The approximate excavation boundary and verification sample locations are depicted on Figure 3. 3.3 SAMPLING PROCEDURES Upon completion of the remedial excavation, Stantec personnel entered the excavation to collect soil directly from the exposed bottom and sidewalls. Collected soils were tightly packed into a laboratory-provided clean 8-ounce glass jar and sealed with a threaded Teflon lined lid. All soil sample containers were sealed tightly and labeled with appropriate identification information (boring number,sample depth,sample collection date,and sample collection time). The samples were then logged onto a chain-of-custody form and placed in a cooler for transport to the laboratory. A copy of the chain-of-custody form is included as Appendix A. Stantec 3.1 PESTICIDE EXCAVATION COMPLETION REPORT a LABORATORY TESTING PROGRAM March 29,2019 4.0 LABORATORY TESTING PROGRAM A total of 13 soil samples collected from the excavation were delivered under chain-of-custody (Appendix A) to Advanced Technology Laboratories (ATL) located in Signal Hill, California. ATL is certified to perform hazardous waste testing by the State of California Water Resources Control Board Division of Drinking Water, Environmental Laboratory Accreditation Program (ELAP). All of the collected samples were analyzed for the presence of OCPs by EPA test method 8081A. Historical analytical OCP data is tabulated in Table T. Verification soil samples are tabulated in Table 2. Analytical laboratory results are included in Appendix A and discussed in Section 5.0. 5 Stantec 4.1 PESTICIDE EXCAVATION COMPLETION REPORT VERIFICATION SAMPLE RESULTS March 29,2019 5.0 VERIFICATION SAMPLE RESULTS Laboratory test results are tabulated in attached Table 2. Complete laboratory analytical results, including QA/QC documentation, are presented on the laboratory analytical reports attached as Appendix A. The following summarizes the results of the analytical testing. The results of the analysis reported concentrations of 4,4-DDE at a maximum of 0.0052 mg/Kg. No other pesticides were detected in the excavation samples. The detected concentrations of 4,4- DDE are well below the US EPA residential RSL of 2.0 mg/Kg. 6.0 WASTE DISPOSAL A total of 313.43 tons (i.e. approximately 200 cubic yards) of pesticide impacted soil was direct loaded and transported to La Paz Landfill in Parker,Arizona. All soils were loaded into covered end dump department of transportation (DOT) approved trucks and transported to the waste disposal facility manifested as non-RCRA hazardous waste. Copies of the tonnage of soil sent to each facility and the associated non-hazardous waste manifests will be provided to the LACFD once received from the disposal facility. Stantec 6.1 PESTICIDE EXCAVATION COMPLETION REPORT CONCLUSIONS AND RECOMMENDATIONS March 29,2019 7.0 CONCLUSIONS AND RECOMMENDATIONS On March 20 and 21,2019,Stantec provided oversight for the excavation of pesticide-impacted soil at previous boring HA-4, where 4,4-DDE was detected above residential RSLs (see Figure 2). Soil samples were collected for the sidewalls and bottoms of each of the two excavations. The resulting excavation measured 45 feet by 40 feet by 3 feet deep. Table 1 provides the historical results of all soil samples collected on the Property for pesticides. Table 2 provides the results of the excavation verification samples. The verification sample results reported concentrations of 4,4-DDE at a maximum of 0.0052 mg/Kg. No other pesticides were detected in the excavation verification samples. The concentrations of 4,4-DDE that were detected in the verification samples are well below the US EPA residential RSL of 2.0 mg/Kg. Conclusion The verification sampling results confirm that the excavation was successful in remediating pesticide concentrations in soil at the Property to below residential RSLs. As a result, Stantec is requesting that the LACFD review the results and provide written approval that site cleanup objectives for pesticides have been met and that grading can commence at the Property. • Stantec 7.1 PESTICIDE EXCAVATION COMPLETION REPORT LIMITATIONS March 29,2019 8.0 LIMITATIONS The conclusions presented in this report are professional opinions based on data described in this report.
