Oishi-BC105) to assist with the fire investigation located at 7 E. Huntington Drive, Arcadia, CA 91006 on 10/16/2021. The location was a 2 story commercial restaurant, with the ground floor comprised of the main operational area. The second story was a maintenance loft area. Upon assignment, I proceeded to obtain a briefing from restaurant staff as to the circumstances surrounding this event. Ernest Tan (Manager) stated that at approximately 9:15pm he was notified by his cook (Joseph Duran) that there was a fire in the kitchen. Mr. Tan stated that he visualized the fire was inside the east wall of the stove area, behind the fryers. The wall he visualized was later determined the hood and duct system. Mr. Tan assisted Mr. Duran in acquiring an extinguisher to discharge in the area; however all attempts to extinguish were futile. In a separate interview with Mr. Duran, I was able to conclude similar statements to the previous comments that were stated by Mr. Tan. I assisted the E105 Captain (G. Couts) to determine the cause and competent ignition within the area of origin, which was determined to be the kitchen on the first floor. The specific area of origin appeared confined to the area within/surrounding the kitchen hood and duct system. No fire movement patterns were observed issuing from the grill top or the fryers; however, the natural gas grill tops and the fryers were stated in normal operation at the time of incident. Additionally, all electricity was on and in normal operation. There were no obvious signs these appliances were associated as the cause of the fire; however radiant and convective heat were present in the area surrounding these appliances that were on and in normal use. Significant grease build up throughout the ducting was present at the time of inspection. It is probable the grease aided as a fuel upon the fire ignition, contributing to rapid fire movement through this area. Natural gas and electrical were present in this area; however further system destruction and forensic analysis would need to be completed to during the specificity of involvement, if any. Additionally, Mr. Duran stated he initially heard a "humming" sound in the ducting, possibly associated with a ventilation fan within the exhaust removal system for the hood and duct. This incident was clearly an accidental cause and the most probable, competent heat sources in the area were determined to be the following: 1. Hood and duct products of combustion/heat build up in the ducting 2. Possibly mechanical/electrical components within the system, aided by products of combustion to accelerate fire growth No obvious signs of electrical failure or natural gas failures were observed, however further disassembly of the system would need to be completed to verify. See attached photos. No investigation case was opened as this was assisting the Company Officer in the determination. ***ADDITIONAL NOTE*** Mr. Tan stated that the fire alarm system was not audibly/ visibly activating at the time of the fire and smoke. The fire control panel stated "open circuit". I notified Mr. Tan that this will need to be addressed before they reopen. I turned the scene over to Erika Laeman (LACo Public Health) upon my completion. She met with Mr. Tan to explain the requirements before they reopen and stated this incident will require the restaurant to coordinate with them, as well as a plan check permit process with the City and County before they reopen for business. E107 arrived on scene and was assigned to back up Fire Attack Group. E107 assisted with extinguishment of fire within hood and duct system. Hood extinguisher system was activated by E107 and Class K extinguishers were used to knock down the bulk of the fire. E107 completed primary search and notified Fire Attack and IC of all clear. E107 was then re-assigned Fire Attack. E107 made access to attic and roof area which houses the hood and duct systems. E107 tied in with Vent Group/T105 and utilized rotary saw to open hood system to ensure knockdown. Foam from Vent Group hoseline was used to overhaul the affected area. Knock Down was called by E107. E107 checked surrounding areas for extension. E107 was released to Available On Scene by BC105/IC and cleared the incident for a medical call in 105's district. NARRATIVE (4) Narrative Title: Fire Attack Narrative Author: Couts, George Narrative Date: 10/17/2021 20:13:48 Narrative Apparatus ID: E105 Narrative: Engine 105 was first on scene to 7 E. Huntington Dr. to find a single story restaurant with moderate smoke showing for the roof. Engine 105 personnel utilized two class K extinguishers to knock the fire down that was in the hood/duct system. After extinguishment, fire attack a well as engine 107 utilized a chainsaw and tools to breach the ceiling to check for extension. Engine 105 Captain assisted Investigator 105 with the fire investigation.
